Clinic Process

To schedule an appointment at our NKC Health campus location, contact (816) 346-7400. To schedule at our Independence location, contact (816) 691-1800.

Appointments are recommended but not required. All scheduled appointments take precedence over walk-ins.

1. When making an appointment, provide:

• Employer name
• Patient Name
• DOB
• Patient email address and phone number
• All services required

2. Forms will be emailed directly to the patient to submit online prior to their scheduled appointment

3. Bring photo ID and authorization form from employer to appointment

Due to the high volume of patients we treat, ONLY patients are allowed in the treatment areas.
