Your Appointment
Let’s make the most of your visit. Before your first appointment, please ask your referring physician to send us your medical records. Try to arrive 30 minutes before your first appointment so we can take care of your paperwork. For follow-up visits, 15 minutes early is plenty of time.
What to Bring to Your Appointment
Having the right information on hand helps us give you the best care. Please bring the following to each visit:
- A current, complete list of your medications
- Any copay or co-insurance amounts
- Your insurance card
Billing & Insurance
We know billing and insurance can feel overwhelming, so we do our best to make it clear and manageable. Read our financial policy to learn about:
- How we handle insurance claims and billing
- What your financial responsibilities are as a patient
- Options for setting up payment plans
You can also review the list of insurance providers we accept and find details about how to pay your bill.
Prescription Refills
There are a few ways you can request a prescription refill:
- At your appointment - Tell your doctor if you’ll need a refill before your next visit.
- From the patient portal - Log in, go to My Medications and select Renew.
- Through your pharmacy or mail-order service - Call your pharmacy or mail-order service for refills, and they will contact us for approval.
To avoid running out, please request refills several days in advance. We respond during business hours, so planning ahead helps ensure you don’t miss a dose.
Patient Registration & Forms
To reduce the time spent at our office, print and fill out any necessary forms in advance. Bring them with you to your appointment.
