You have a lot on your mind, and we’re here to guide you from day one. Knowing what to expect and how to prepare for a cancer appointment can help — and having everything ready to go will reduce any anxiety you feel going into your first appointment with us.

What to bring

• Insurance card
• Lab results, test reports and imaging (CD) of any recent testing performed at another facility
• List of medications
• Name and contact information of your referring physician and primary care physician
• Past medical records related to your condition
• Photo ID (driver's license, passport, military ID or other government-issued documentation)
• Questions for your doctor
